Medical information & sources
Effective October 25, 2026 · Version 1.0
Medicine Notes is a personal medicine journal. Its records, reminders, scan suggestions and AI descriptions are for organization and general information.
Public source: MedlinePlus.gov
The optional guide search uses MedlinePlus Connect, a public service of the U.S. National Library of Medicine, part of the National Institutes of Health. The result opens the original MedlinePlus drug-information page. That page identifies its publisher, updates and copyright. Some drug information is licensed from the American Society of Health-System Pharmacists (ASHP). Medicine Notes links to it and does not reproduce the drug articles.
Independent service, no endorsement
Medicine Notes is not affiliated with or endorsed by MedlinePlus, NLM, NIH, ASHP, or Apple. Their names identify the source or platform. Their content and trademarks remain theirs. We do not create or verify external medical pages and cannot guarantee their accuracy, completeness, availability, or suitability for your location or circumstances.
Check the exact medicine
Search uses the name you enter. It may return a different formulation or no match. English U.S. drug information can differ from products and guidance in your country. Check the active ingredient, strength, form and local label. Consult a doctor or pharmacist before making medical decisions. Do not start, stop or change treatment based on this app or a linked page.
AI descriptions have a different source
An AI-generated description comes from Apple’s on-device language model, not MedlinePlus and not a live medical database. The app marks generated descriptions. Neither a model’s output nor its guardrails guarantee medical accuracy. You can edit or remove the text, or leave the description empty.
No diagnosis or clinical calculation
The app does not diagnose, monitor a medical condition, recommend treatment, calculate a clinically safe dose, or check drug interactions. Stock estimates, unit arithmetic and bundle shortcuts depend on your input. They do not establish safety. The app is not an emergency service and does not send your journal to a clinician.
Your rights remain
These explanations describe the limits of the app and external information. They do not exclude liability or rights that applicable law does not allow us to exclude. Apple’s standard license agreement applies to the app license.
